Location: New York City, NY

Salary: $130.00 USD Hourly - $140.00 USD Hourly

Description: The Judge Group is looking for two Family Nurse Practitioners to perform Health Risk Assessments throughout Kings County/the Brooklyn NY area! This can be done either as a part time position or full time. For part time, the minimum days of working is 1 day per week. Payment is per assessment and is $140 per assessment, this is a fixed rate. Mileage is also reimbursed at 65 cents per mile.

Job Description:

 -  Perform health risk assessments which are essentially history and physicals done within the patient’s home or on occasion in nursing home or other health facility.

 - Document acute, chronic, and long-term healthcare issues through patient encounter in EMR system; Documentation must be accurate, high quality, and completed in a timely manner.

- Ensure all appropriate consent forms are signed and dated. 

- Interact frequently with patients, coders, schedulers, and supervising physician.

 - Educate patients and/or patient’s family on chronic medical conditions, preventative care, and medication adherence compliance.

- Complete referrals to case management, nursing, pharmacy, etc. as needed 

- Participate in clinical meetings and compliance trainings which are primarily web based; however, provider will occasionally be required to attend on-site training

 - Will NOT be required to prescribe medicine nor order any lab or diagnostic testing.

- Occasionally need to obtain samples for diagnostic screening tests such as finger stick for HgbA1C, LDL, and Lead levels; urine analysis may also be required based on client need. 

- Responsible to review patient records for past and present health issues.

-  Collaborate with the supervising physician and other members of health care team.

- Interpret laboratory data and diagnostic testing when available to assist in diagnosis of medical conditions

-  Complete ADL and Fall Risk evaluation, Depression and Nutritional Health Screening, BMI evaluation, and medication reconciliation

 - Perform other duties as assigned by supervising physician such as perform examinations using equipment for diabetic retinal exams, bone density, etc.

Qualifications: 

- Family Nurse Practitioner 

- Must be board certified by the ANCC or AANP

 - Must have active/unrestricted New York NP license 

- New graduates will be considered 

- Must be able to be credentialed by Medicare, Medicaid, and other Private Insurance Companies

 - Proficiency with various computer based applications such as MS Office (Logging into programs, MS Word, MS Excel, MS Outlook, general use of mouse and navigation of different programs, typing) along with utilizing various websites to perform the required duties. 

- Strong background/interest in primary care and preventative medicine is a plus 

- Must have use of own car with adequate automobile insurance

 - Highly motivated, self-directed professional with strong organizational skills and comfortable working independently on a daily basis

 - Must have home internet connection
