Job Description:
Are you a dedicated Nurse Practitioner seeking a flexible role where you can make a meaningful impact on patients' lives? Our client is looking for experienced Nurse Practitioners to join our team in conducting in-home health assessments. This per diem position offers you the chance to work independently, with compensation for each assessment, plus mileage reimbursement.
Responsibilities:
- Conduct comprehensive health assessments in patients' homes, including medical history reviews, physical exams, and screenings for chronic conditions.
- Provide education on medication management, disease processes, and preventive care.
- Identify and document diagnoses to support care management and active treatment.
- Collaborate with primary care providers to communicate findings and address potential gaps in patient care.
- Flexibly schedule your appointments throughout the week, with no on-call, weekends, or holidays required.
Minimum Requirements:
- Completion of at least 30 in-home assessments per month.
- Valid Nurse Practitioner license in the state of Maine.
- Board Certification from AANP or ANCC in Family, Adult-Gerontology, or a related specialty.
- Minimum of 1 year of clinical experience; experience with geriatric populations preferred.
- Strong communication skills and a commitment to delivering compassionate care.
- Reliable transportation for travel within the assigned areas.
- Ability to manage various dwelling conditions and engage with patients comfortably in their home environments.
