Springfield, VT: We seek a skilled PRIMARY CARE Family Nurse Practitioner or Physician Assistant to join a respected healthcare organization in beautiful southeastern Vermont (near the New Hampshire border). Join the medical staff of a multi-site nonprofit FQHC/community health center that is innovative, patient-centered, and emphasizes quality, access and affordability for all. Integrated behavioral health professionals and care coordinators are embedded in their Patient-Centered Medical Homes.

Become part of an upbeat physician led primary care team offering services to patients of all ages - including some complex patients - on an outpatient only basis. Use an integrated approach to provide safe, therapeutic care in a holistic and systemic way, seeking to provide overall care management and useful patient and family education. Physicians support the APPs, serving as resource and mentor as needed. Enjoy a convenient 4 day/week schedule (40 hours) in a modern facility with a collegial team-based environment.
