Job Description

Provides services for the congestive Heart Failure (HF) program to include inpatient HF consultation and care coordination, assure guideline directed care and HF metrics are met, improve efficiency and effectiveness of care. This will lead to earlier discharge, less readmission. increasing outpatient volume of care and expanded access to coordinated and specialized heart failure care.

• Identify and access patients at ED arrival or after admission by filtering through automated lists for appropriate cases and other means identifying admitted patients with heart failure in the hospital
• Alert the multi-disciplinary heart failure care team to presence of patient
• Respond to HF consultation requests
• Round with HF team
• Oversee application of evidenced based and algorithm therapy including obtaining recordings from implanted invasive hemodynamic monitors (CardioMems ; if present) so as to optimize therapy for all heart failure patients regardless of category and degree of heart failure
• Ensure patients admitted to floor have consults placed for Nutrition-HF, Pharmacy- HF, PT-HF within 24 hours of admission
• Ensure alert SW-CM team aware of Heart Failure status of patient to begin early referral process for home support services
• Ensure the patient the patient’s family has full understanding of their disease state, life style and diet modifications needed and medication compliance
• Early identification of CCU HF patients for readiness of educational intervention
• Assist ER and team in early identification and safe discharge of patients from ER with mild to moderate ADHF and ensure outpatient follow up of patients within 1-2 days of ED discharge
• Coordinate care amongst physician team, Nutrition, Pharmacy, SW-CM team, Palliative and Nursing to ensure adequate education for patient and care-taker empowerment
• Provide patients with appropriate support referrals in the outpatient setting including, but not limited to, other subspecialty follow-up for comorbidities, free nutrition courses, Home Health Aide Services, Visiting Nurse services.
  
  

Minimum Qualifications

• A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED); and
• Current certificate as a Nurse Practitioner issued by the NYSED; and
• Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
