Overview
Our client is seeking an experienced Nurse Practitioner or Physician Assistant to perform in‑person, no‑cost annual preventative health risk assessments for Medicare, Medicaid, and Affordable Care Act members. The provider will conduct visits within members’ homes and must be comfortable working independently, traveling within a defined geographic radius, and maintaining high‑quality clinical documentation.
This per diem role offers flexible scheduling and requires a minimum of 15 completed assessment appointments per month.

This per diem role offers flexible scheduling and requires a minimum of 15 completed assessment appointments per month.
Key Responsibilities
- Perform in‑person annual preventative health risk assessments in members’ homes
- Successfully complete required self‑paced and instructor‑led training
- Provide a minimum of 15 appointment slots per month
- Ensure high‑quality assessments that meet all satisfaction and quality expectations
- Document and submit chart notes within 24 hours of each visit
- Respond to charting queries within 48 hours
- Maintain effective communication with clinical leadership
- Attend periodic meetings and training sessions
- Safely enter and work within members’ personal living spaces
- Lift up to 20 lbs as needed
- Bend or kneel for extended periods when required during assessments
- Master’s or Doctorate degree from an accredited NP program (FNP, AGNP, ANP)
- OR Master’s degree from an accredited PA program
- Home health experience within the last 5 years
- Strong computer skills and proficiency with electronic medical records
- Understanding of CMS and ACA risk adjustment models
- Ability to document assessment findings accurately and thoroughly
- Strong verbal and written communication skills
- Ability to maintain HIPAA compliance and patient confidentiality
- Ability to make sound clinical decisions within established guidelines
- Ability to adapt to changing work conditions and responsibilities
- Ability to anticipate and resolve member, provider, and health plan issues
- At least 1 year of in‑home assessment experience (highly preferred)
- Current RN and NP or PA licensure in the state of practice
- Board certification (ANCC, AANP, or NCCPA)
- State Medicaid identification number
- Medicare PTAN number
- Current BLS certification
- Valid, unencumbered driver’s license
- Proof of adequate auto insurance coverage
