Do SNF discharge summaries written by APRNs need to be co-signed by MDs? My facility is saying this is the case, although I can't find this requirement in any of the Medicare regs?

The discharge summary doesn't have to be cosigned but the page one and the face to face should be.

Kathy, what state are you in? They are trying to claim that it's a state reg (not federal) that the attending has to sign them. Face to face co-sign I am aware of, but didn't know any actual notes had to be co-signed!

What I do when faced with one of these comments, I insist on asking to see the documentation. What really happens is that it is not a regulation but how things are always done

In MA, the d/c summaries do not have to be signed. Because the VNA apparently needs an MD sig on Pg 1 order, I usually write my name and title, followed by "for Dr _____". That apparently satisfies the Pg 1 issue. For the face to face, the MD has to sign. Always they are signing but not filling then out. Then the staff tries to get us to fill it out. I hand them back to staff and advise them to fill out in their Medicare meeting. We don't need more busy work!
